Sport of Life Liability Waiver

 This program will be led by Sport of Life Inc., an affiliate of Prescribe Outside.

By checking “I agree” in the event registration, you acknowledge and agree to the terms below.

1. Participant Eligibility:

I confirm that I am at least 18 years of age and fully capable of understanding and consenting to the terms outlined in this document.

2. Assumption of Risk:

I am aware that participating in physical activities conducted by Sport of Life Inc. and Prescribe Outside involves inherent risks. These risks may include, but are not limited to, physical injury, illness, permanent disability, and death. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by me as a result of participating in any activity.

3. Medical Consent:

I consent to receive medical treatment deemed necessary in the event of an injury, accident, or illness during my participation in activities. I agree to be financially responsible for the costs of such treatment.

4. Release and Waiver of Liability:

I hereby release, indemnify, and hold harmless Sport of Life Inc., Prescribe Outside, their officers, agents, employees, and other participants from any and all claims, demands, actions, and rights of claims for damages, injury or loss to person or property which may be sustained or occur during participating in any activities, whether or not they are foreseeable or contributed to by my negligent acts or omissions.

5. Acknowledgment of Understanding:

I have read this waiver and release form, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement voluntarily and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law.
